
Move 2 Independence can support someone with getting back to living well at home, following discharge from hospital. It has been developed by a charity called Somerset Activity and Sports Partnership (SASP). SASP helps people find ways to move more that work for them to promote good health and happiness and are pleased to be working with hospitals across Somerset with this new service.
Regular movement is important for overall wellbeing and has many benefits both while staying in hospital and when a person returns home. Getting back to doing what they want, need and enjoy at home and in the community are the foundations of Move 2 Independence, which puts the person's goals at the centre of the service.
SASP offer a range of movement resources and programmes that are adaptable, making them suitable for a range of health conditions and symptoms someone may be living with following a stay in hospital. The person is in control of how they use them to benefit their recovery but all opportunities are low impact, focus on different types of fitness and may even be enjoyable!

If you would like to join the Move 2 Independence service, please speak to a health professional on the hospital ward, or the Discharge Facilitator, to request a referral. The referral needs to be made via the Single Point of Contact (SPOC) referral form, please get in contact if you require further advice or guidance about making a referral.
