The Enhanced Access service is available for eligible patients registered with one of the following GP practices:

- The Meadows Surgery
- Summervale Surgery
- Langport Surgery
- Church View Surgery

Physical Activity on Referral for Universal Credit Recipients
If you are in receipt of Universal Credit and are a registered patient in one of the surgeries listed below, you may be entitled to a FREE 12-week ProActive gym membership with Chard Leisure Centre:
