Since launching in 2022, ProActive Somerset's Physical Activity on Referral Scheme has become an important part of Somerset's preventative health offer, supporting healthcare professionals to connect patients with safe, structured opportunities to become more physically active. Designed for people living with a range of health conditions, the programme helps participants improve their health, manage symptoms and build confidence to remain active long term. For many, it provides a supported pathway into physical activity that they may otherwise feel unable or hesitant to access independently, offering specialist guidance in a safe and reassuring environment.
The continued success of the programme reflects the commitment of healthcare professionals and referral partners across Somerset. To date, the service has received 5,709 referrals, with 1,884 individuals successfully completing the 12-week programme, including 474 completions since October 2025. Through referrals from health and care settings, people are being supported to access specialist exercise programmes tailored to their individual needs.
Delivered through a network of 25 accredited leisure providers, ProActive continues to strengthen links between healthcare, physical activity and community-based support. This collaborative approach helps ensure residents can access local services that support both their immediate health needs and longer-term wellbeing.
The impact on participants continues to be encouraging. Among those completing the programme, 62% reported improved wellbeing, 56% improved their blood pressure, 53% increased their physical activity levels, and 41% achieved improvements in weight management. Positive outcomes have also been reported in areas such as strength, balance and overall health, demonstrating the significant role that physical activity can play in supporting prevention, rehabilitation and long-term condition management.
The programme is also helping to reach priority populations, with over half of referrals coming from people aged over 59 and referrals received from some of Somerset's most deprived communities. The most common referral reasons include obesity, osteoarthritis, hypertension, mood disorders and lower back pain, highlighting the breadth of conditions for which physical activity can provide meaningful support.
Alongside participant outcomes, ProActive has invested in workforce development by funding specialist qualifications for exercise professionals, helping to expand Somerset's capacity to deliver high-quality physical activity on referral services.
Thank you to all our healthcare professionals, referral partners and providers who continue to champion physical activity as part of the health and care pathway. Together, we are improving access to support, reducing health inequalities and helping more Somerset residents lead active, healthier lives.
