About Wellington Health and Wellbeing Coach

We are looking to recruit a full-time Health and Wellbeing Coach, with a physical activity focus, to work within the Taunton Deane West Primary Care Network. They will provide 1:1 support to people referred to them by team members, group education/coaching workshops and physical activity sessions at community level.

The role of Health and Wellbeing Coach centres on improving patient and community wellbeing - physical, mental, social and emotional - through supporting access to social prescription and increasing activity levels. The Health Coaches work in partnership with the PCN and local community they ensure appropriate support is available to individuals, families and carers to improve the quality of their mental and physical health and wellbeing.

The successful candidates will have good communication, negotiation and people management skills and act with compassion, empathy and integrity. Experience or familiarity with person-centred coaching conversations is desirable.

The post holder will work with a diverse range of people and communities and support them to develop their knowledge, skills and confidence to actively look after their own health and wellbeing through positive behaviour change. A compassionate and practical understanding of the barriers facing those from varied social, economic and cultural backgrounds, or living with long term health conditions, including mental health issues, will be important.

The Health and Wellbeing Coaches roles are non-clinical.

This is a full-time role with some flexible working, there will be a requirement for some evening and Saturday availability. A core training package, as well as ongoing development opportunities are integral to the role.
