Chard, Langport and Ilminster Health & Wellbeing Service
Our Chard, Langport and Ilminster SASP Health and Wellbeing service (Enhanced Access) supports people struggling with their weight or living with a long-term health condition to access physical activity in order to promote and protect their health. If you are struggling with your weight, living with a long-term health condition, at risk of diabetes or just ready to take control of your health, the qualified team at Chard Leisure Centre can give you the free support and access to the leisure centre at key times to make healthy changes to your lifestyle.
The Enhanced Access community work is available to anyone registered with one of the following GP practices:
- Ariel Healthcare
- The Meadows Surgery
- Summervale Surgery
- Langport Surgery
- Church View Surgery
Your qualified Instructor will help guide and work with you through your current health issues and ability, to help you find ways to incorporate more free health-giving activities into your current lifestyle and help you gain the confidence to build a healthy future. You can gain access to participate in a wide variety of free and fun activities both in and out of the centre in evenings and Saturdays.
With 92 classes and 73 swim sessions delivered, over 70% of patients improved their fitness throughout the 4-week period.
To set up an appointment, please email: firstname.lastname@example.org and reference Enhanced Access and whatever GP surgery you have been referred from. You will then be referred to a class instructor who will meet you and talk you through everything from there.
